By booking my appointment, I hereby release Reverence Hair Studio, LLC, its stylists, colorists, and employees, from any responsibility and/or liability concerning the application, processing, and/or consequences of any chemical/non-chemical procedure of my hair.
By booking my appointment, I hereby give consent to the following services: Haircuts, Hair Coloring, Corrective Coloring, Blonding, Permanent Waving (Perms), Hair Treatments, Hair Styling, Brow Waxing, Brow Tinting, Lash Tinting, Cosmetic Applications, Lash Applications, and Hair Extensions.
By booking my appointment, I understand and give my consent to the following:
– I have made my stylist aware of any previous chemical/non-chemical processes on my hair and understand that what has previously been done to my hair may alter my final result.
– My stylist has the right to refuse service if the process is going to sacrifice the integrity of my hair and will always do their best in recommending alternative solutions for service.
-If I am under 18 years of age, my guardian will be present with me at the time of service. I understand that without parental consent, my service will be refused.
By booking my appointment with Reverence Hair Studio, LLC, I assume all risk of injury and harm resulting from the service specified herein and I agree to release, defend, indemnify, and forever discharge Reverence Hair Studio, LLC, from all liabilities, claims, damages, costs, and expenses, or any action due to loss, damage, injury, or death that might incur resulting from the chemical/non-chemical service.
